**REMINDER**
ALL ATHLETIC FORMS AND FEES ARE DUE BY THE FIRST PRACTICE. IF NOT RECEIVED, STUDENT ATHLETES WILL NOT BE ABLE TO PARTICIPATE UNTIL RECEIVED.
NEW ATHLETIC DEPARMENT PROCEDURES
1. ALL forms and the $60.00 sports fee MUST be turned in at the first practice or the student athletic will NOT be able to participate until they are received. NO EXCEPTIONS! If you are not due for your physical, please fill out the waiver form stating when you will be getting your physical. Athletic forms are only due the first sport season the student is playing a sport except for the New Health Concern form.
2. There is a new form that needs to be filled out if there is a new health concern throughout the school year. It is the responsibility of the student athletes Parent/Guardian to make NCS aware of any change in condition that warrants attention. This form is available on the school website under Athletics.
