Student Forms are required with the August 1st payment. 

Student Class List will be available at the Parent Orientation Meetings on Tuesday, August 24th.  Student Class List will not be released without the required forms for this school year.  Students will not be allowed to attend school without all forms being on file the first day of school. 

Forms are available in the school office if you can not download or print the forms.

• Allergy Alert - Required for all preschool - 8th grade students
• Application - Required for all new students requesting enrollment
• Child Information Record - Required for all K-8th grade students – this is a Web-Fill Form – complete the form online and print
• Kids Club Enrollment & Agreement - Required to receive the discounted rate for before and after school care.  Students not enrolled will be charged the higher drop in rate fee.
• Kindergarten Questionnaire - Required for all Kindergarten Students
• Little Lambs Childcare Application - Required for all children wishing to enroll in the daily childcare.
• Medical Authorization - Required for all K-8 students requiring any medications during school.  All medication, prescription and over the counter, must be stored in the school office and administered by the school office personnel.
• Milk & Meal Pre-order Ticket Form - There are no single milk and meal purchases.  Milk & Meal Tickets should be pre-ordered or your account will be charged for the appropriate ticket upon purchase of a milk or meal.
• Pastoral Reference Letter - Required for all new K-8 students
• Preschool Student Emergency Card - Required for all preschool students
• Preschool Placement Agreement Form - Required for all preschool students
• Preschool Questionnaire - Required for all preschool students
• Shared Time Internet Release - Required for all 4-8 grade students
• Shared Time Student Information - Required for all 1-8 grade students
• Student Physcial Form - Required for all New Student entering preschool - 8th grade & current 6th grade Students. This form can also be used in place of the Sports Physical.
• Volunteer Drivers From - Required to be completed and on file 1 week prior to driving students on a field trip.
